eGFR = 142 × min(Scr/κ, 1)α × max(Scr/κ, 1)−1.200 × 0.9938Age × (1.012 if female)

κ = 0.7 (female) or 0.9 (male); α = −0.241 (female) or −0.302 (male). Units: mL/min/1.73 m².

Cockcroft–Gault (CrCL)
CrCL = ((140 − Age) × Weightkg × (0.85 if female)) / (72 × Scr)

Weight may be Actual, IBW (Devine), or Adjusted (AdjBW = IBW + 0.4×(Actual−IBW)).

Tip: When labels specify CrCL, use Cockcroft–Gault. The “un-indexed eGFR” multiplies eGFR by (BSA/1.73) to give an approximate mL/min value for comparison.
